Brush Health Services
Use forms listed below to specify which immunizations are required to enter each grade level. Parents may also contact the school clinic for copies of the forms.
Students with Food Allergies

The Brush School District recognizes that many students are being diagnosed with potentially life-threatening food allergies. To address this issue and meet state law requirements concerning the management of food allergies and anaphylaxis among students.
Parents, the school administrator and the school nurse will develop a health plan using the Student Allergy and Anaphylaxis Health Form.
District Hearing/Vision Screening
Vision and hearing screening is conducted for students in their schools according to the Colorado Department of Education guidelines. These guidelines indicate that students in kindergarten, first, second, third, fifth, seventh and ninth grades, and exceptional needs students receive vision and hearing screening. Because of the important connection between a student's academic achievement and their vision and hearing, parents are encouraged to observe their student and seek a medical evaluation for changing or significant concerns. The school clinic aide may be contacted for information regarding health and community resources available to parents.
Section 504 of the Rehabilitation Act of 1973 is Congress’s directive to schools receiving any federal funding to eliminate discrimination based on disability from all aspects of school operation.
The health and wellness of our students is as important as academic achievement, and the district has a wellness policy and a health education policy.
Accident and Dental Insurance
Our students’ safety and well-being is our greatest concern and we want them to have medical care in case of an accident. We encourage parents and guardians to enroll their children in accident and medical insurance programs. There are several low cost plans available including K & K Insurance Group. Review their premium costs here and see a flier and enrollment form. Check with an insurance company for additional plans.
The State of Colorado offers Child Health Plan Plus (CHP+), which is for families that earn too much to qualify for Medicaid, but cannot afford private health insurance. The State of Colorado Website provides additional information about medical insurance programs.
The district School Medicaid Program serves to enhance or expand health and related services for all students. This program provides the opportunity for parents to meet with
program staff and enroll eligible students in CHP+ or Medicaid insurance. Please contact the School Medicaid Program with question at 970-842-4794.

The Department of Health Services provides lists of websites and community resources to help encourage healthy lifestyles. These documents are provided in both English and Spanish.
